If you’re a Delta Dental Medicare Advantage DHMO provider, new members from our partner health plans may be assigned to you or choose your practice as their primary care facility. Explore this quick reference guide for helpful information.

We’ve partnered with the following Medicare Advantage health plans to provide dental coverage to their members in 2025:
 

What else do you need to know?

Patients may visit your practice stating they have coverage from one of the above health plans, and present their Medicare Advantage plan name and/or ID card. This means that they’re likely to have Delta Dental. Log into Provider Tools to check their eligibility — the patient’s Medicare Advantage plan ID number can be used to locate them within Provider Tools.



Services for Medicare Advantage DHMO patients are only covered if you’re part of Delta Dental’s Medicare Advantage DHMO network. Please check eligibility and benefits prior to treatment. A member must be assigned to your office prior to treatment to receive coverage under their plan.



Finally, benefit coverage for your patients may have changed. Before discussing treatment options with them, log into Provider Tools to verify their benefit details for the current year.
 

Set clear expectations with a pre-treatment estimate

To increase patient satisfaction with your practice and their health plan, it’s a good idea to provide a treatment plan and submit it to Delta Dental for a pre-treatment estimate. This helps patients understand their coverage, and any associated cost. It’s also helpful to review all services with your patients beforehand so they know what’s covered, and ask them to sign a financial responsibility form before you perform any procedures.
