  2. Dental expenses are excluded if incurred in connection with any dental procedure started before enrollment in the DeltaCare USA Program.
  3. Orthodontic treatment must be provided by a licensed dentist. Self-administered orthodontics are not covered.

DeltaCare USA networks includes access to over 75,365 dentist locations; Delta Dental Fact Sheet 2020. Dental services received from any dental facility other than the assigned Contract Dentist, or a preauthorized dental specialist (oral surgeon, endodontist, periodontist, pediatric dentist or Contract Orthodontist), except for Emergency Services as described in the Evidence of Coverage, are excluded.
